- 1.Complete the report before the officer goes off shift, while memory is fresh.
- 2.Write in the first person and in plain past tense: what you saw, what you did, in order.
- 3.Separate observation from conclusion. Describe behavior; do not label motive or diagnose anyone.
- 4.Attach photos, video stills, and witness statements as numbered exhibits referenced in the narrative.
- 5.Route to the supervisor for review the same day, then to the client per your contract's notification window.

Before you file it
- Every time in the narrative matches the DAR
- No opinions, diagnoses, or legal conclusions written as fact
- Names spelled from an ID or the person's own spelling where possible
- Photos saved with the report number in the filename
- Law enforcement case number recorded if a report was taken
- Supervisor reviewed and signed
- Client copy sent within the notification window in the contract
